Extremely sensitive skin

I am having sensitive skin on my palms and fingers of both hand and foot .The difficulty is that when I am consistent with water for more than a minute the outer layer of skin starts getting teared from the rest and gets extremely dry.Can I know why it happens? Is it because of the water or soap or any other reasons? And is there any solution to prevent this?

My question is what medication are you on? Prednisone can do this and this is called prednisone skin. Otherwise, you might want to see if you have autoimmunity: cushing's and some form of hyperthyroidism can do this, as well as scleroderma/crest.

I react similarly when washing dishes without gloves, swimming, or showering with hot water because of eczema on my fingers and elbows. I keep it at bay with pure lanolin. Prior to caring what when on and in my body, I used a steroid cream from the doctor. My results with lanolin are actually better than they were with the prescription cream and I don't transfer the steroids to my children through touch!
