I started changing my diet a year ago this week! Like a lot of folks, I began with a low-carb approach that gradually morphed into a paleo diet with whole foods and starches.
Overly focused on eating clean when good sleep and stress reduction is even more important.
Believing that VLC weight loss is sustainable.
Eating too much animal products, particularly beef.
Eating too much healthy fats because it tastes good and everyone talks about it.
Largely ignoring shellfish, crustaceans and sea vegetables.
Giving up sugar and sweeteners.
Limiting consumption of nuts.
Stopping myself from proselytizing paleo to friends and family.

April was 1 year for me. I went low-carb Primal immediately in an attempt to control my glucose levels. I went off diet once and paid dearly for it. It is how I learned I was gluten intolerant and figured out gluten was the source of the heartburn I had suffered from for 20+ years. I did low-carb Primal for 4 months, lost 40 lbs and gained control of my diabetes, then learned I was pregnant. I upped my carbs to 100-150 each day to stay out of ketosis, but had to use insulin to maintain control of glucose levels. I still ate strictly Primal, but enjoyed more tubers and fruit, which I am still having a hard time coming off of. I delivered in April and, even with the pregnancy, am down 80 lbs from when I started.
I am continuing to lose weight, am exercising more, am no longer anemic (but still at the low end of "normal," and have normal blood sugar levels. Right now, I am trying to find the balance between eating enough to maintain my milk supply, keep my sugar levels in check without medication (not using Metformin or insulin now), and continue to lose weight (I still have 80 lbs or so to go).

August 17 will be one year for me. I have been very strict and have added 18 lbs to my figure. Nutrition numbers have improved but still have UC symptoms. Hoping they go away the longer i eat this way. The most frustrating thing was reading robb Wolf's book and seeing how his UC went away in like a day! I was expecting a mircle like that but he probably fluffed his illness up to sell books.

First 3 months on paleo, lost 18 lbs, got down to 6% bodyfat, never felt better. Then I got into a habit of eating pseudo-paleo foods, and trying to count calories, and justifying cheats. Gained back 8 lbs. Then realized I felt better when I ate just real foods, and lots of it. Lost the 8lbs, back to 6% bodyfat, won a couple of benchpress meets, and its been great ever since.
