any good book on the history of illness ?

The strongest point behind the paleo diet is that the illnesses of civilisation: cardiovascular problems, cancer, diabetes, etc, were either non-existent or very rare among hunter gatherers. A couple of months ago I read about a discovery of atherosclerosis in Egyptian mummies http://www.nytimes.com/2009/11/24/health/24heart.html (which, incidentally, does not contradict our view since Old Egypt was an agricultural, not paleo society). My question is whether you know of any good reference on the history of those diseases, meaning some book that covers topics such as when was cancer, diabetes or cardiovascular illness first found ( I mean based on modern evidence even if Ancient World people were unable to diagnose it) whether it had serious prevalence in old Rome, or in the Middle Ages, etc, etc. It would be interesting to know, from a modern perspective, what do we know on the history of illness, to compare it with the different diets that people followed historically.

Taubes GCBC and why we get fat cover this information. It has more breadth than depth but lots of references and lots of studies of various cultures
