Paleorrhea- not fun

I started Paleo 4 months ago and about 6 weeks in, started getting diarrhea, which was always accompanied by fleeting painful stomach cramps and a sudden need to go. And I mean sudden! I was going sometimes up to 20 times a day. Food didn't stay in my body for more than 20 minutes. One day, I accidently took a double dose of probiotics (40 bill) and got some relief: my stools hardened a bit and I didn't need to go as often. If I am consistently taking my probiotics, I can at least function in the world, but I am curious why all of a sudden Paleo has altered my gut flora so much that I'm now dependent on the probiotics? I'm also worried about other damage to my gut because I still experience the painful stomach and intestinal cramps and a sudden need to go (every time).

I eat lean meat, eggs, fish, vegetables, fruit, nuts and seeds. My ratio is about 50f/30p/20c.

Some things I tried: adding rice to diet, cutting out all acidic foods, adding dairy, decreasing nuts, decreasing fat, adding soluble fiber foods, increasing protein. Nothing worked. I occasionally (1-2x per week) have black tea or decaf coffee, local honey, and red wine. I tried omitting all of these but that didn't help either.

I'm frustrated because I never had gut issues before Paleo and I am experiencing so many health benefits from it that I would like to stick with it (or a modified version of it).

Second that. If its as severe as that, do see a doctor; you won't be getting many nutrients with transit time like that.

My friends and family thought I had Paleorrhea when I first went Paleo and wouldn't shut the hell up about The One True Way. :) (apologies -- I hope you feel better soon.)

It's 5 days long, with every day you have 1 tbsp psyllium husk with 2 cups water after waking and before bed. Day 1-3 is 10-12 cups fruit juice/day, day 4 has 3 cups yogurt plus 5 cups juice, day 5 like day 4 plus 5 boiled eggs. Drink plenty of water also, all week. This is a fasting protocol used by famous personal trainer Vince Gironda, to resolve digestive problems in his clients.

You may be allergic to a food, like I was to coconut, which caused a little weirdness until I figured it out, but other than that, I can't see how this is paleo related. It is more likely some bug you caught.

Ok, so. Your diet does seem fine. If you're getting this type of diarrhea AFTER all this time of eating so well, then it's not dairy/allergies, or IBD/IBS or anything else like that. IMHO, the way you describe it, it has the clear marks of an infection. Go to your GI immediately and ask for stool tests for h.pylori, C.Diff, protozoa, etc. No, you can't fix these with diet alone most of the time, because these eat anything you eat (the "good" bacteria mostly go for the carbs, bad ones can eat anything you eat).

Also, in the meantime, add fermented foods, bone broth, and home-made goat kefir (home-made kefir is known to be able to kill SOME bad bacterial infections even when antibiotics fail to do so).

Do not wait any longer, you should have gone for a stool test for a long time now.

A week long fruit juice-only fast with psyllium husks permanently cured my lifelong chronic diarrhea. Afterwards, I was able to go back to the same diet that caused problems before and tolerated it fine. I have no idea how or why, but if you're desperate it might be worth trying.

I don't believe any of the supposed explanations for how these work, "colon cleansing" etc is probably total BS, but they do seem to have some benefit for some people by an unknown mechanism.

Lower protein and increase fat. Only eat 1 oz of nuts a day. Fattier meats!!!

So I just finished the week-long juice fast that healthEngineer described, and my digestion is better than it has been in months--as good as it has ever been in my life. Previously I assumed chronic gas and diarrhea were just normal. It got better when I cut out gluten a year ago, but for the last 2 months my stools have been very loose, bordering on diarrhea, which started after I broke a 24 hour fast with a huge steak & potatoes meal.

I followed the protocol he described in his comment, and frankly I'm shocked at the improvement. Poops are regular and super-clean. So nice.

Like healthyEngineer said, the usual explanations for cleansing fasts are unconvincing, but there must be some unknown mechanism. I suspect it may reset your gut ecology, like a dietary clear-cut.

what was your previous (solid stool) diet composed of?

i also notice very soft stool when being on high protein/high fat with low carb. but the minute i bring in some bread or pasta things get bulky and hard.

but i also noticed frequency changed to 1 movement per 2 to 3 days.

the only thing that makes me go that much is my ulcerative colitis. have you had a colonoscopy done or seen a GI doctor? are you still currently going this much? have you lost a ton of weight? without a doubt, you aren't absorbing nutrients at all if the food you're eating goes through you so quickly. you need to get some bloodwork done to check your inflammatory markers, electrolytes, blood count, vitamins, etc.

what i would do for the time being is get rid of eggs, nuts/seeds, nightshades. you know, all the things on the autoimmune protocol. maybe you have an intolerance to one of these foods. before i started on paleo, i was going 15-20x a day so i know how you feel, trust me. i've gotten down to 4-8x a day which may seem a lot to most, but it's way better than how i was previously.

If you were running to the toilet 20 times a day then you probably have either a serious medical condition or very very Poor food instincts. The closest I can relate to that is when I was drinking 12 raw egg yolks at a time for a couple weeks. I stopped when my stools became ridiculous which in hindsight was probably a mild case of salmonella. You need to take a serious look at your diet and find out what the problem is, either that or go to the doctor; that's not normal.
