Is it true that fat is first lost from the extremities and last from the mid-section?

Where you lose fat is highly relative. It not only depends on genetics, but hormones, chemical balance, thyroid function, and type of fat.

Hormones dictate this one. If they an not in balance you may not burn any fat from anywhere on your body.

High insulin, high cortisol, high estrogen - these will inhibit fat loss especially from the belly and butt.

Charles Poloquin's Biosignature method 1: http://www.charlespoliquin.com/Education/Biosignature.aspx claims you can read your body fat to determine which hormones are out of whack.

Chasing around these downstream hormones is a sisyphean task if you ask me. It seems that the whole shebang is controlled by leptin. So start reading here if you are up to it. It's dense, but great. link text

That is true for me and my own personal experience. My toes are svelte. You may have noticed people who lose weight lose it in the face first. That's all controlled by hormones, the details of which are beyond me.
