Has anyone had a lot of success with clearing acne on a paleo diet? I'm thinking about going semi-paleo again. Is this a good option or I need to try something different other than this?

Yes! Flax seeds contain phytoestrogens - substances with acts like the hormone estrogen in our bodies. So it is possible for flax seed to upset your hormone balance which can result in acne breakouts (more so if it's consumed regularly in your diet, but it's still possible through topical application to the skin). I get the same problem when I consume products with a lot of soy because soy is also an estrogenic food.

In your case it is necessary for understanding the type of acne you are facing with?
What type of skin do you have? Whether it is dry, oily.
You might have being using some different types of products for acne. What products you have used?
There are different types of acne for example:

While following the paleo diet you should also take care and carefully select the products you are using for acne. Products to be used along with the Paleo diet must be prescribed by health care professional.
For example: Disfiguring case of nodular acne a product with combination of Tetracycline and Isotretinoin ingredient for acne will be recommended by health care professional.

I have being recently reading on Hormonal Acne. May be information related to it may help you.
The primary drivers of the hormonal acne are oil secretion due to due to androgens, which are male sex hormones.
Following are some of the categories in which these type of acne are divided:
Acne due to:
1) high testosterone from PCOS
2) low estrogen from low body fat levels
3) high progesterone from general hormone imbalance etc
I would like to recommend some of the following tips that may help in curing the acne by following paleo diet.
Try to remain stress free. Stress acts as an inflammatory agent.
Low carbohydrate diet is also one of the reasons for acne. Sufficient glucose store is important for skin healing
Dairy: Many people at least anecdotally least respond to dairy with acne more strongly than any other food.

There are more restrictive diets for this issue. Just try to remove any possible irritants/allergins for a period of time, then add them back in one at a time. You could also try improving your skin care roitine I.E. Cut out harmful products, try to retain skin flora, moisturize appropriately (or not).
Usually people remove Dairy, gluten, eggs, nightshades maybe nuts.
