I figured FODMAPS are the cause of SIBO, acid reflux and constipation. Am I right to make this generalization or could there be other reasons/causes for the combination of symptoms?

I would disagree with that generalization. The main thing with FODMAPs is that they cause gas, not the symptoms you describe.
One discussion here:

She has a protocol that combines paleo and FODMAPS with ideas for treating SIBO.
I don't buy the line about starch being an irritant to the gut. I'm not the only one:
Chris Kresser has a great article about this as well:

Soluble fiber: gentle to the gut; think cooked carrots, sweet potato, parsnips.
Insoluble fiber: harsh on gut; leafy greens, broccoli, cauliflower, brussel sprouts.
When I was dealing with candida, at my worst, I was avoiding vinegar, mushrooms, starch and all fruit. While I'm not 100% sure I've killed it all off, my worst symptoms are gone and I eat fruit if I feel like it, and I sure as shit am not going up my sweet potatoes again!
I've never heard ANYONE claim that starch is "irritating" to the gut. But starch can feed pathogenic bacteria and fungi and cause them to overgrow. Starch = shitload-of-glucose = more food for the critters
i think starches are a big problem in digestive issues so i would avoid those (they are legal on FODMAPS).
